Hello Everyone! Here are the instructions for how to reserve your 2011 Exhibitor Booth for the 24th Cape Cod Symposium on Addictive Disorders!!!
RESERVE YOUR EXHIBIT BOOTH
Click this link complete your reservation: 2011 CCSAD Exhibitor/Sponsor Registration.
(you can also go to www.ccsad.com - click on Exhibitor Only, then Exhibit Purchase)
Select your Organization from the drop down menu on the right. If it is not listed, select the third option to be added to the waiting list. If you wish to purchase Marketing Items only - no exhibit please select the second option
NOTE TO THOSE EXHIBITORS SPONSORING A SPEAKER: Please select that you will “pay by check” so that I can go in and give you your $200 exhibit discount. I will re-send you a corrected invoice.
If for some reason you will not be able to be with us this year, please let me know so that I may release your exhibit space to someone on the waiting list!

Scroll through the screen to select any additional items (ad in our onsite brochure, material in the participant tote bags, Banner ads on the website or in-house in the exhibit hall and hotel TV Channel, etc). We also have several sponsorship options that we would love to partner with you on! Please select any that you are interested in, including electricity. Your exhibit comes with the pipe and draping (booths) draped table (booths and table exhibits), chairs, carpet, waste can and small signage. You will not need carpet or any other items.
Please fill in the information as you would like it for public dissemination in the Company Information Section.
Accept the Insurance and Liability Policy (this is required by both the hotel and our insurance companies that exhibitors provide their own liability protection.)
Fill in the exhibit/marketing/billing contact info. This is for me to notify you of any deadlines or need for materials.

STAFF
2 Complimentary registrations come with the exhibit booth and include CEUs. You will receive an email in July with instructions on how to register your staff!!
PAYMENT
You may either pay online with a credit card or print an invoice and send a check. The Exhibit will be held as Pending for 30 days after receipt of this contract. If payment is not received within 45 days of receipt of contract, the exhibit will be marked Available and contract will be null and void. The Exhibit will be marked Reserved when payment in full is received
You are finished! You will receive an email confirmation of the process. You will also receive an email in July to complete the staff registration
